Inlays and Onlays

When a filling isn't enough but a crown is too much — lab-crafted porcelain restorations that repair big damage while keeping your healthy tooth.

A repair between a filling and a crown

What Are Inlays and Onlays?

Sometimes a cavity is too large for a filling to fix reliably — but enough healthy tooth remains that full crown coverage may not be necessary. That's exactly what inlays and onlays are for. Both are custom restorations crafted in a dental lab from strong porcelain, then bonded precisely into your tooth. An inlay fits within the grooves between the cusps of a tooth, like a puzzle piece. An onlay goes further, covering one or more cusps — which is why it's sometimes called a "partial crown."

Why choose an inlay or onlay over a filling?

The Benefits

A filling is shaped and hardened directly in your mouth. An inlay or onlay is made outside the mouth and bonded to the prepared tooth. The choice depends on the damage and the support your tooth needs:

  • Support for selected larger repairs — the design and material are chosen for the damaged tooth
  • Designed for long-term use — lifespan varies with the tooth, restoration, bite, and home care
  • Custom fit — a lab-made restoration is checked and bonded to the prepared tooth
  • Coverage where needed — an onlay can cover weakened or damaged cusps
  • Conservative preparation — may preserve more tooth structure than a full crown in suitable cases
  • Tooth-colored porcelain — selected and shaped to blend with nearby teeth

The Right Tool for the Job

An inlay or onlay may be suitable when a cavity or fracture needs more support than a filling, but the tooth does not need full coverage with a crown. Examples include a large failing filling, a cracked cusp, or decay across the chewing surface. The aim is to repair the damage while preserving healthy tooth structure.

What to expect

The Inlay and Onlay Process
1

Exam and Diagnosis

An examination, photos, and appropriate X-rays help us assess the damage. We’ll explain why a filling, inlay, onlay, or crown may be suitable.

2

Preparation and Scan

After numbing, we prepare the tooth conservatively for the restoration. A digital scan records its shape, and a temporary restoration protects it.

3

Lab Crafting

A dental lab mills your restoration from strong porcelain, with a shade selected to blend with your tooth and a shape designed for the prepared area.

4

Bonding

We verify the fit and your bite, then bond the restoration permanently. We explain how to care for it and when to return for a checkup.

Caring for your inlay or onlay

Aftercare and Lifespan

Brush twice daily, clean between teeth daily, and keep the checkups recommended for you. Call if sensitivity persists or worsens. Checkups let us assess the restoration and surrounding tooth. If you grind at night, ask whether a custom nightguard would help protect your teeth.

Common questions about inlays and onlays

Why not just get a bigger filling?
The amount and location of remaining tooth structure matter. If a tooth needs coverage over a weakened area, an onlay or crown may be more suitable than a larger filling. We’ll explain the options for your tooth.
Are inlays and onlays covered by insurance?
Your plan may help cover an inlay or onlay, but benefits and limits vary. We’ll check your coverage and explain your estimated share before treatment.
Does the procedure hurt?
It feels much like getting a filling — done under local anesthetic, with sedation options available if you're anxious. Mild sensitivity for a few days afterward is normal.
How many visits does it take?
Typically two: preparation and scanning at the first, bonding the finished restoration at the second, usually about two weeks apart.
